To expedite the first appointment process, please print each of the forms noted below, then complete and bring them with you to your first session.

• Health Information Privacy Notice
• Communication Consent Form

Please remember that services provided by Preferred EAP are free of charge to eligible participants.

However, if you make an appointment and then fail to keep it, or cancel an appointment with less than 24 hours notice, that session may be deducted from the sessions for which you are eligible.

So, if you need to cancel an appointment with us, please call well in advance.
